Here is a lovely PNG base/template of the progress report from the back of the SO packaging. I was debating over giving you guys the PNG and the PSD, but I decided that the PNG was just easier-less chance my TMing will be removed too easily.
So since it’s a PNG the “outlines” and colored stuff are clear (the "bg" of the image is the dA color), so you can just paste it on top of your choose color theme. The white text you should black out-I included it so you could see where to put your text. The “Name” should also be the same color as your color theme-I left it white here so you could see it and where it goes.
-For the “Name” text I used the font Monga at 20 pt, stretched 125% vertically and 75% horizontally
-For the written text I used the font Minion Pro at 12 pt, 14-18 pt leading (depending upon the space and amount of text I had), stretched 125% vertically and 71% horizontally
Not to say you have to use the same fonts/font setting I did, I just thought it would be helpful. You might also need to move the Headmistress’ “signature” if you put a lot of text down there, which is perfectly fine. I had also thought about making a MMS version, but then I realized we don’t have a principal yet!

I pretty much recreated all of this from a scratch off of a SO Deena box scan. I did use the circle and MH crest stamps though from =
MHScreamQueen’s sticker pack since it was quicker and they’re so clean and crisp. I point this out to show how much time I spent on it, so please do not remove my TM up top, nor forget to credit/link back, ok? Please and thank you guys ^^
And ok, if you guys want the PSD as well I might put it up later then, I just figured everyone can use a PNG, while not everyone can use a PSD.
